Reminder: Most Reconsideration Requests for PRF Phase 4 Payments Due by May 2

Providers who believe their Provider Relief Fund (PRF) Phase 4 payment was not calculated correctly have until 8:59:59 p.m. (PT) on May 2 to request a reconsideration from the Health Resources and Services Administration (HRSA). 

Providers who received a payment determination on April 13 will have until May 28 to submit a reconsideration request. Providers who have a pending payment determination will have 45 days from when the payment determination is received to submit a reconsideration request.    

Providers should carefully review the PRF Phase 4 calculation methodology before submitting a reconsideration request. The reconsideration process is intended to review Phase 4 payment decisions where the provider received less than it expected (including instances where no payment was received). HRSA will not accept reconsideration requests that would require a change to methodology or policy. It will also not consider instances where a complete application was not submitted (e.g., HRSA was unable to validate the applicant’s taxpayer identification number). HRSA will review the original, submitted application again and will request clarifying information from the provider, as needed. Providers may not revise or correct their submitted application.   

The PRF Reconsiderations Request Form must be completed in DocuSign. Because this form requests information in the original submission, it is essential to accurately and completely fill out all fields, using the same information originally provided. This will allow HRSA to retrieve your application materials for review.  

The form also includes sections for updated contact information, as appropriate, and the reason for the request. Providers will not be able to submit additional documentation with the completed request form. HRSA will review only the original submitted application and will request clarifying information, as needed. 

Submission of a PRF Reconsideration Request does not guarantee approval or adjustment to a provider’s payment. All reconsideration determinations are subject to the availability of funds.
