HRSA Releases Application Instructions, Template, Updated FAQs for Phase 4 Provider Relief Fund and American Rescue Plan Rural Distribution

The Health Resources and Services Administration (HRSA) has released additional materials that support the application process for the Phase 4 Provider Relief Fund (PRF) and American Rescue Plan (ARP) rural distribution. These include a sample application and application instructions. Of note, the instructions also include links to “technical assistance” documents that providers are encouraged to review prior to beginning their application.

Based on HRSA’s application process flow chart, providers  are encouraged to begin the application process early to allow time for the IRS to validate the applicant’s tax identification number (TIN). According to the HRSA document, this can take up to 10 days.
