Deadline for Providers to Return Electronic Claims Agreements for Medi-Cal Rx Is July 31

On June 30, the Department of Health Care Services announced an extension and reminder to all participating Medi-Cal Rx providers and billers to submit their completed Medi-Cal Rx Telecommunications Provider and Biller Application/Agreement Form by July 31. If a provider’s form is not received by July 31, they will NOT be able to continue to submit electronic point-of-sale claims until it has been received.  

The application/agreement form must be completed in blue ink and submitted with original signatures to:

Rancho Cordova, CA 95741-0610 

The original due date was April 30, and providers were initially notified of this requirement in December 2021 and reminded again in March 2022.
