Acute Psychiatric Hospitals Should Review Adverse Event Reporting Policies

What’s happening: The California Department of Public Health released All Facilities Letter (AFL) 25-15 on April 29, reminding acute psychiatric hospitals about existing requirements to report adverse events and unusual occurrences.  

What else to know: Hospitals must report adverse events no later than five days after an event is detected, unless it constitutes an ongoing, urgent, or emergent threat to the welfare, health, or safety of patients, personnel, or visitors; in those cases, the adverse event must be reported no later than 24 hours after detection. The AFL also provides specific examples of adverse events that facilities must report.
