CHA would like to remind members that Provider Relief Fund (PRF) Period 3 reporting closes on Sept. 30 at 8:59 p.m. (PT).
Providers who received one or more General and/or Targeted PRF payments exceeding $10,000, in the aggregate, from Jan. 1, 2021, to June 30, 2022, are required to report on their use of funds in reporting period 3 using the PRF Reporting Portal.
