Guidebook on Strategic Opportunities in Post-Acute Care Market Updated; Save the Date for March Webinar

The CHA Center for Post-Acute Care has released an updated Productive Partners: Hospitals and the Continuum of Care guidebook to help hospitals and health system leaders align strategic thinking around post-acute care and engage with post-acute providers. 

The comprehensive guidebook provides information, tools, and other resources to help hospitals implement effective strategies and partnerships with post-acute providers, including internal and external rehabilitation, skilled nursing, long-term care, and home health. 

The 2021 release reflects the current post-acute care landscape and addresses how post-acute care partnerships may be critical to a hospital’s clinical and financial success and can accommodate diverse payers, respond to changing payment requirements, and address shifting consumer perspectives.  

In addition, a recording of CHA’s Developing Effective Partnerships in the Post-Acute Space webinar featuring faculty from Premier, Inc., is now available. The webinar expands on critical content from the guidebook and discusses how to establish successful post-acute care partnerships. 

A follow-up webinar, Strategic Post-Acute Partnerships – A Deeper Dive, is scheduled for March 16.
