Providers may submit requests to report late due to extenuating circumstances for Provider Relief Fund Reporting Period (RP) 2 to the Health Resources and Services Administration (HRSA).
The forms must be submitted from May 2 to May 13 at 8:59 p.m. (PT). Interested providers should review and submit the Request to Report Late Due to Extenuating Circumstances Form.
Non-compliant providers will not be required to return funds until after the request to report late due to extenuating circumstances processes for RP1 and RP2 has closed. Providers who submit a request will be notified by HRSA if their request is approved or denied.
