What’s happening: The Collaborative Healthcare Patient Safety Organization (CHPSO) — a division of the Hospital Quality Institute (HQI) — is hosting a webinar on Feb. 19 at 11 a.m. (PT) to discuss empowering high utilizer patients through individualized comprehensive care plans, including partnership with the California Office of Diversity, Equity, and Inclusion.
What else to know: HQI is an approved continuing education (CE) provider by the California Board of Registered Nursing and will provide CHPSO members who attend the webinar with an opportunity to earn CEs of 1 contact hour (Provider No. CEP16793). Registration is open.
Presentation topics include:
- Utilizing risk stratification and data analysis to determine the correlation between high utilizer patients and avoidable readmission rates and cost of utilization
- Developing individualized care plans that encompass medical, social, and psychological aspects, and input from DEI champions resulting in improved health outcomes and reduced hospital readmissions
- Integrating tools and enhancements in the Electronic Medical Record system to facilitate the creation of effective and efficient visualizations and easy access to care coordination notes and care plans
- Identifying common barriers to effectively adapt the discussed framework and strategies
