The California Department of Public Health recently released new All Facilities Letters (AFL) on licensing and certification applications and fiscal year 2023-24 health care facility license schedule.
AFL 23-24 outlines revisions to the Licensure and Certification Application (HS 200) and Applicant Individual Information (HS 215A) forms. The revised forms were effective starting July 1. It is important to note that HS 200 is for initial license applications and for any other license changes.
AFL 23-22 provides information on the 2023-24 license renewal fees for health care facilities and the process for renewing your hospital’s license. The fee schedule is available on the CDPH website.
