Earlier this month, CHA shared feedback with the Department of Health Care Services (DHCS) on its Draft Population Health Management (PHM) Strategy and Roadmap 2022.
CHA’s comments urged DHCS to ensure the following:
- A realistic implementation timeline is established
- Providers receive the necessary resources to carry out new requirements
- Any new mandated information technology systems are interoperable and easily integrated into clinical care processes
- DHCS’ PHM efforts are coordinated with other overlapping state initiatives, such as the California Department of Health & Human Services Data Exchange Framework
