The CHA Center for Post-Acute Care has released an updated Productive Partners: Hospitals and the Continuum of Care to help hospitals and health system leaders align strategic thinking around post-acute care and engage with post-acute providers. The comprehensive guidebook provides information, tools and other resources to help hospitals implement effective strategies and partnerships with post-acute providers, including internal and external rehabilitation, skilled nursing, long-term care and home health.
The 2021 release reflects the current post-acute care landscape and addresses how post-acute care partnerships may be critical to your hospital’s clinical and financial success and can accommodate diverse payers, respond to changing payment requirements and address shifting consumer perspectives.
